SCHOOL ENROLLMENT FORM

The school enrollment form can be completed instead of providing the three proofs of residency if the child is of school age and attending school in the league charter area. The school enrollment form cannot be used if the player is attending a home or cyber school or a school outside the GSV area. The form only needs completed one time as long as the child continues to attend the same school, (i.e. grade 1 at Avis Elementary would be good until grade 5 at Avis Elementary, if the child moves to the middle school a new form is needed)
